Do I need a doctor's referral or prescription to come to physical therapy?
Yes, you will need a doctor's referral or prescription. In California, you can be evaluated by a physical therapist without a referral, but you cannot be treated. We can't bill your insurance without a referral from a physician, even for an evaluation.
My doctor's referral is for treatment at another clinic. Can I still come to Mt. Eden?
Yes, you have the choice to go anywhere, depending on your medical insurance coverage.
What will my first visit be like?
You will arrive early and fill out the registration papers, which includes a short medical history. Please bring a list of current medications, your photo ID, and your health insurance card. If you are having knee pain or if you've had surgery, your physical therapist will want to see the surgery site, so wear appropriate clothing.
You will be taken to a private treatment room for your evaluation. During the evaluation, your physical therapist will interview you about the nature of your pain or dysfunction, when it began, how it affects your daily routine and you will be tested for such things as muscle strength, range of motion, gait abnormalities and posture irregularities. After the evaluation, a complete report will be faxed to your referring physician, which outlines the proposed plan of care.
After the evaluation is complete, you will begin physical therapy. Please plan to stay at least an hour for your first appointment. Follow-up appointments can range from one-half hour to 45 minutes.
Will my treatment records be available to me and to my doctor?
We provide comprehensive evaluation reports, progress notes, and and maintain daily chart notes, chronicling your path to recovery and documenting the extent of your injury, your pain levels, and any limitations to your daily activities. If you were involved in an accident or a work injury, these records are crucial to your case.
I don't have insurance. Do you have special rates for patients who pay cash?
Yes. We charge cash paying patients reasonable rates which are similar to the fees our PPO contracts pay. Please call the office for rates.
You are not in network with my insurance. Why and when is it advisable to go to a provider who is out-of-network?
When a clinic accepts contracts that don't pay enough per visit to provide you with quality treatment, they are forced to make up lost income in other ways: treatment by less than qualified personnel, extending the number of treatments, or treating groups of patients at the same time. Ultimately, you would want the most efficient, effective treatment by highly qualified physical therapists. If you have been to another physical therapy practice and are frustrated by lack of progress, inattentive treatments, care given by underqualified employees, or simply if you would like a second opinion, you may want to consider going "out-of-network." Carefully choosing where you are treated may provide you with a speedier recovery or might provide you with more insight into your condition.
I'm not sure if you're in network with my insurance? How can I tell?
First, we're glad to check your insurance for you. You can also investigate your coverage by telephoning your insurance company. You might find our contract several ways: Through our business name, Mt. Eden Physical Therapy Center; through the owner's name,Jean-Pierre Viel; or through our tax payer ID: 94-3063718.
Why is in-house billing important?
It is our goal to inform you of your physical therapy benefits, your deductible and your co-payments. (However, we urge all patients to check their health insurance benefits before coming in). When we file your claim for you, we make sure that your claim is processed according to your health insurance plan and our contract. Claims are filed promptly and your billing is mailed promptly. We do our best to determine a patient's financial responsibility, before they embark on a series of appointments. No one enjoys a high balance at the end of treatment! Above all, we will advocate for you with your insurance company should there be a need.
